Introduction. Recently, a completely new concept of health care – «Medicine 4P» – has become increasingly widespread. Its name is derived from four fundamental principles: personalization of predictii, of prevention and of participatively. «Medicine 4P» is a format based on an individual approach to each patient, the purpose of which is the preclinical detection of diseases and their risk factors, as well as the devel-opment of a set of preventive measures. Given the relevance of urolithiasis in urological practice, the relationship of its development with metabolic disorders, it is important to assess the possibility of applying the principles of the concept of «Medicine 4P» for the organization of medical care for patients with this disease.

Methods and Results. The paper describes in detail the principles of «Мedicine 4P» on the example of management of patients with urolithiasis, a comparison of curative and preventive models of medical care on the example of assistance to patients with urolithiasis. The article presents the world and domestic experience of the concept of «Medicine 4P», demonstrated the possibility of applying this approach in the organization of medical care on the example of urolithiasis.

Conclusion. Application of the principles of «Мedicine 4P» will not only reduce the incidence of ICD, but also improve the health of citizens and reduce economic costs by reducing the number of patients requiring high-tech medical care
