Introduction. The problem of urolithiasis (ICD) retains its high relevance throughout the world due to the high prevalence among the population, frequent recurrences of the disease. Among the risk factors for the development of ICD, a violation of urodynamics in the urinary tract is an important endogenous pathogenetic risk factor for the development and recurrence ofICD.Acommon cause of violation of urodynamics of innate nature isthe intrarenal type of pelvis.When urodynamics are impaired due to the intrarenal type of pelvis with the formation of secondary stones and frequent exacerbations of pyelonephritis, along with the removal of the stone (s), an adequate correction of urodynamicsis an important and necessary condition forsurgical treatment. Due to the anatomical features, the correction of disturbed urodynamicsin this category of patientsis a difficult task, but a fundamentally important and indispensable condition for successful treatment.

Materials andmethods. The operative method of treatment“Magnifying plastic of the pelvis of the intrarenal type” developed for the treatment of 36 patients with a complex clinical form of urolithiasis was applied. When using the proposed method, an extrarenal pelvis of physiological volume and shape is created, thereby achieving an adequate correction of impaired urodynamics.

Results. Using the proposed method of surgical treatment in 36 patients, ensured a quick and stable rehabilitation of the patient and improve their quality of life. During the dynamic observation of the operated patients and the ICD metaphylaxis,with follow-up periodsfrom 1 to 11 years, recurrence of stone formation was observed in 3 (8.3%) patients.
