The phosphodiesterase inhibitors are first-line treatment for the most types of erectile dysfunction. Sildenafil citrate is the first and the most studied member inhibitors. However, research on the effectiveness and safety of sildenafil in separate groups of patients and continuing to this day. Such high interest to this drug caused by large heterogeneity of patients population taking sildenafil.

In a review on the basis of the modern literary data and results of own researches one of the aspects least studied in the domestic literature – pathophysiology of low urinary tract symptoms (LUTS), connected with a role of sexual hormones (androgens) in pathogenesis at men and women is considered. With the years the expressed growth of LUTS and noctiria frequency in population irrespective of a gender is marked, and one of key mechanisms is the age deficiency of sexual hormones, promoting development of bladder hypoxic ischemia.

Aim: evaluation of the efficacy and safety of the new uroselective alpha-blocker silodosin (Urorec®) in the monotherapy of the patients with the combination of two illnesses: benign prostatic hyperplasia (BPH) and chronic non-infectious prostatitis (CNIP).

Aim of the investigation: to evaluate the characteristics of the microcirculation in the prostate and gonads of the adolescents with isolated varicocele and varicocele with the pelvic congestion using laser Doppler flowmetry (LDF).

We can now even witness the situation, when only 36.9% patients, who were referred to a physician, receive a proper ED treatment. The concerns are related more to a safety than to an efficacy. It is being told that these medications have severe cardiovascular consequences and carry a risk of addiction.

The scientific data of the last years shows that increased level of the testosterone in the blood is not a main factor of the development of benign prostatic hyperplasia (BPH), though the exogenous supply in big doses is used for the induction of BPH in the experimental animals. Moreover the growth of the frequency of the BPH is linked to the low level of the testosterone, which is a part of the ageing and a consequence of the pathological conditions, such as metabolic syndrome or diabetes.

Phosphodiesterase type 5 inhibitors (iPDE-5) are the first line medications for erectile dysfunction in men. Given that all of them are highly selective, nevertheless they can have effects on the other isoforms of phosphodiesterase (including type 11), which are located in different tissues. Some experimental works show that influence on PDE-11 could affect spermatogenesis. Nevertheless, there is no common opinion whether if fertility affected or not and how.
